    The first screen shot you posted shows a memory management bug in rEFInd; however, it's from an old version of rEFInd, so it's uncertain if the bug is the cause of the problems with the later version. Also, the message is from the Tianocore library, so it doesn't pinpoint where in rEFInd the bug is. It would take a lot of back-and-forth effort to diagnose the problem.     VirtualBox does a poor job of "remembering" its EFI settings across boots, particularly if you completely shut down the VM between boots. This makes it work much like a computer with a broken NVRAM.     If you're not booting anything in BIOS/CSM/legacy mode, I recommend you disable rEFInd's scanning for BIOS/CSM/legacy boot loaders by adjusting the scanfor line in refind.conf. Specifically, you should remove the hdbios option.

    I recommend you check on Mac forums for information on downloading the Mac installer and writing it to a USB flash drive or DVD, then install from that. Unfortunately, I don't have step-by-step instructions handy. A big caveat: The instructions I've seen for this have all assumed a working macOS installation, so you may need to use another computer, dig a while to find something that'll work from Linux, or figure out how to adapt the instructions yourself.

    This problem used to be common, but is now pretty rare. See here for information on it: http://www.rodsbooks.com/refind/installing.html#sluggish Also, if you've installed unnecessary EFI drivers (in EFI/refind/refind_ia32) , you should uninstall them. At best, they'll add to your startup time; and at worst, they can hang. One more point: If you've got lots of leftover or experimental boot loaders, you should clean them out; scanning the directories and constructing boot loader entries takes time....
