A communities platform that helps coordinated care network, track outcomes, identify service gaps and at-risk populations and most importantly empower members
About Unite Us
Our platform is constantly evolving to better serve our users and the communities they support. We are secure, flexible, and can grow with you, which is why innovative communities and institutions across the country have partnered with Unite Us. The first step of a client’s journey is often screening to identify as many of their co-occurring needs as possible. We’re screening tool agnostic, so we’ll configure your questions with our algorithms to help you predict risk and give you immediate steps on how to address the needs of your clients. Go beyond searching for resources and referring your clients. Since the platform is embedded within every organization in your network, you’ll know if they got there, what happened every step along the way, and the specific outcome that improved their health. Organizations receiving referrals need the right information to best serve each client.

"Platform BROKEN for over 1 year " Edited 2023-09-30
Pros: Nothing, software doesn’t work, software engineers don’t understand the business their software supports, software negatively impacting business of customers, looking for new platform due to severe defects staying open for over a year. These people don’t really know what they are doing. In 30+ years of working with COTS software, Unite Us is by far the worst.
Cons: Support is bad, Software engineering team doesn’t understand business they support, zero creativity in solving problems, major defects Unite Us created still open after 1 YEAR!
Overall: Worst experience working with commercial off the shelf software I have ever experienced. Sending feedback to NYSDOH to stay away from this company in support of the Medicaid 1115 waiver.
