Bookingpad is an open source booking engine. It is designed to create simple booking sheets in an easy and intuitive manner. To install please read the Tomcat Installation instructions. This should be transferable to any standard java servlet/jsp container. System Requirements are Java 1.6(though should compile with 1.5) and a Servlet/JSP Container (Tomcat 6.0 recommended). Scripts and configuration are provided for Mysql and Postgres though any database supported by hibernate should work.
