Minor changes to manual .

Giving a meaningful error when abolish_table_subgoal is performed on a predicate that is

Oops -- forgot one of the load_dyn(c) tests.

Added a couple more tests of load_dyn(c).

Adding a few more tests of dynamic loading with modules along with dynamic loading of
trie indexed coded.

Revision of dynamic loading section to include load_dyn_gen/2.
(Please help me with a better name).
The various options for this predicate are explained, while the various incarnations of
load_dyn and load_dync are explained in terms of option choice for load_dyn_gen
(again, better name?). Even though load_dyn_gen (name?) is implemented just a fancy switch
to load_dyn(c) I think the explanation is now simpler and clearer.

As part of the manual revision I updated the discussion of static vs. dynamic code
in the System chapter, and added a discussion of canonical syntax in the Syntax chapter.

Making ensure_loaded/3 standard.

Removed obsolete FactFlg in load_dync. Also put all constants in usermod in load_dyn(c) (even if standard_symbols).

Fixed some uses of incorrect defined constants: tag instead of token constants. (Note didn't change .xwam).
