#29 SQL Debug missing instance id parameters

When you set the logging level to DEBUG each SQL
command is displayed including the parameters. But in
some cases the instance id is not displayed for the select.

e.g.
About to execute SQL statement: SELECT rv.recordid FROM
e_group rv WHERE rv.recordid = ? ;

should also display

Parameter 1 value: 0-0020-rv
