[L2tpd-devel] Call/Session flow control commit

OK...confirmed interoperability with both the Cisco 3000 VPN
Concentrator and our RedBack SMS 500 with the changes for taking flow
control out on the call/sessions.

So...summary is that it touched six files (yikes!), but seems to work
with at least the above two pieces of equipment.  I *think* based on my
reading of the code that its conceptually correct too, at least this
specific aspect of it.  There are, no doubt, other bugs still there,
even relating to handling of sequence numbers and such on
calls/sessions, but at least this is one thing cleaned up.

Anyway...the commit has been done, so let me know if you see anything
wiggy with it.
