#64 Deny adding New cards with illegal format.

A card with empty 'From:' field is illegal. CardView
dialog [+Add] should refuse accepting new cards with
broken syntax.

Also, when loading a Deck that has card(s) with empty
'From' field, instead of reporting a failure, fill the
empty field with a UNIQUE warning message and let the
user fix the problem.
