Interesting.  Your note template looks fine to me; I notice that it uses 
<FREQ> twice, for p4 and p5.  But when I look at the generated CSD, it 
looks to me as if the second frequency is missing.

Steven, is this possibly a bug in blue, that only the first <FREQ> token 
is substituted, and the second one is ignored?
